Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to identify emotional, social, and environmental strengths and challenges faced by patients and their families.

• Develop and implement tailored intervention plans utilizing appropriate social work methodologies to address the unique needs of each patient.

• Collaborate with healthcare professionals across disciplines to ensure alignment in care planning and support for patients’ ongoing needs.

• Facilitate effective discharge planning by coordinating with consulting physicians, third-party payers, and other healthcare providers to arrange suitable post-discharge resources.

• Provide education and counseling to patients and families on adaptation strategies concerning illness and life changes.

• Engage in crisis intervention and provide therapeutic services tailored to individual, group, and family dynamics.

• Deliver training and educational support to other healthcare professionals on psychosocial interventions and best practices.
\n
• Participate in clinical evaluations for oncology patients, assessing conditions such as depression and anxiety, and offer short-term counseling as necessary.

• Be available for on-call rotations to address after-hours consultations and support urgent patient needs.
\n
• Serve as a community resource, offering educational sessions, consultations, and other supportive services that strengthen community resources for cancer survivors.

Patient Population / Client Focus\n

Your primary focus will be on patients within the hospice setting, many of whom are confronting serious illnesses, including cancer and other life-limiting conditions. You will be working closely with individuals and families from diverse backgrounds, requiring a deep understanding of cultural sensitivities and the emotional complexities tied to end-of-life care.

Qualifications\n

• A Master's degree in Social Work from an accredited program.

• Current licensure as a Clinical Social Worker (LCSW) in New York State.

Compensation and Benefits\n

This full-time LCSW position offers a competitive salary ranging from USD 70,000 to 100,000 annually, commensurate with experience and qualifications.
