Key Responsibilities\n

• Manage an individual caseload of patients referred by the Social Work Service Department, employing thorough evaluations to ascertain emotional, financial, and cultural factors that may impact the overall treatment and care plans.

• Conduct assessments and develop crisis intervention strategies, providing immediate support to patients and their families during high-stress situations.

• Lead healthcare team collaboration to discuss protective service issues, life care decisions, and advance directives, ensuring that complex psychosocial situations are addressed efficiently.

• Facilitate communication among healthcare professionals, community services, and families to ensure a comprehensive approach to patient care, with a focus on respecting patients' rights throughout the process.

• Supervise assigned clinical staff, including Social Workers, MSWs, and interns, offering guidance and mentorship to enhance their professional growth and service delivery effectiveness.

• Coordinate care transition activities for patients, especially focusing on children and high-risk cases, while engaging with relevant community agencies to ensure proper follow-up.

• Implement care strategies to reduce hospital readmissions by connecting patients with necessary resources and preventative services, ensuring efficient use of available funds.

• Maintain professionalism and continuously educate team members on psychosocial issues and the critical role of social work in patient care, fostering a more informed healthcare delivery environment.

• Attend and contribute to specialty team rounds and patient care conferences, bringing a holistic view to case discussions.

• Be on call on a rotational basis to provide urgent support to the medical team and assist with crisis situations as needed.

Compensation and Benefits\n
