Key Responsibilities\n

• Conduct comprehensive psychosocial assessments using interviews, observations, and medical record reviews to identify emotional, social, and environmental factors affecting patient recovery.

• Provide individualized counseling and emotional support to patients and their families, addressing issues related to coping, adjustment, and overall mental health.

• Collaborate with a team of healthcare professionals—such as physicians, nurses, and occupational therapists—to create tailored treatment plans that align with patients’ psychosocial needs.

• Guide patients in navigating community resources, including support groups, vocational rehabilitation services, and housing assistance to promote their independence and well-being.

• Facilitate discharge planning and ensure smooth transitions from detoxification to subsequent care settings, ensuring that necessary supports are in place.

• Participate in regular team meetings to share insights on patient progress and adjust treatment plans as needed.

Patient Population / Client Focus\n

The LCSW will work primarily with individuals going through detoxification for substance use disorders, addressing both their clinical and psychosocial challenges.
