Case Manager, Social Work - Inpatient

Kaiser Permanente seeks an Inpatient Social Work Case Manager to support children and families at our Washington, DC facility. You will conduct psychosocial assessments, coordinate discharge planning, and connect families with community resources while collaborating closely with physicians, nurses, and allied staff. Drawing on a culture of innovation, you will leverage modern tools and electronic health records to streamline care and documentation, contribute to quality improvement initiatives, and help design user-friendly processes that enhance patient and family experience across the continuum of care.
