Case Manager, Social Work

Kaiser Permanente

Kaiser Permanente seeks an inpatient Case Manager, Social Work to support patients at our Washington, DC facility. In this role, you will complete psychosocial assessments, coordinate discharge planning, and connect patients and families with community resources. You will collaborate closely with physicians, nurses, and care teams to support safe transitions of care, address psychosocial barriers, and advocate for patient needs. You will maintain accurate clinical documentation, provide crisis intervention as needed, and contribute to a supportive, patient-centered hospital environment.
