Work Environment

In this role, you'll primarily work in patients' homes, offering a unique, personalized environment that aligns with our core mission of patient-centered care. The position encompasses a Monday to Friday schedule, comprised of 40 hours per week. Key Responsibilities

• Conduct thorough evaluations of patients referred by physicians to develop individualized treatment plans that address their specific needs.
• Implement and adapt treatment strategies using evidence-based practices to facilitate functional recovery, mobility, and pain management.
• Employ therapeutic exercises, manual therapy techniques, and modalities that align with each patient's therapeutic goals and the physician's prescribed plan of care.
• Educate patients and their caregivers about the treatment process, exercises, and self-management strategies to promote sustained recovery.
• Collaborate with an interdisciplinary healthcare team, sharing insights and strategies to maximize patient outcomes and improve overall care effectiveness.
• Monitor patient progress and adjust treatment plans as necessary to ensure optimal recovery paths.
• Maintain clear and thorough documentation of patient care and progress in compliance with legal and ethical standards.
• Engage in community outreach and program development initiatives to promote physical health and awareness about physical therapy.

Patient Population / Client Focus

This role primarily serves a diverse array of patient populations, including individuals recovering from orthopedic injuries, pediatric patients facing developmental challenges, adults with neurological conditions, and athletes requiring sports medicine rehabilitation. Each patient presents unique challenges, and our commitment to personalized care ensures that each therapy session is tailored to meet their specific needs and goals.

Qualifications

• Bachelor's degree in Physical Therapy, with a Doctorate (DPT) preferred.
• Valid State License to practice Physical Therapy.
• Certification in Basic Life Support (BLS) from the American Heart Association.
• Exceptional interpersonal and communication skills, both written and verbal.
• Proficient in word processing and utilizing electronic health records (EHR) systems.
• A strong desire to connect with patients on a personal level to foster rapport and trust.
• A proven ability to build and maintain professional relationships within the healthcare community.

Preferred Experience

While a Bachelor's degree is the minimum requirement, a Doctorate in Physical Therapy is preferred. Experience within a home care setting or familiarity with treating a wide range of conditions is highly valued. Proficiency in various therapeutic techniques, including manual therapy and modalities such as ultrasound or electrical stimulation, will be advantageous in effectively meeting patient needs.

Compensation and Benefits

We offer a competitive salary range of USD 80,000 to 100,000 per year, commensurate with experience, along with a comprehensive benefits package.
