This position offers the flexibility of remote work, allowing you to effectively manage your schedule from the comfort of your home while remaining committed to our hospice center clients. Your primary working hours will be Monday through Friday, from 9:00 AM to 5:00 PM. Key Responsibilities\n

• Serve as a primary liaison between clients, families, and outpatient healthcare providers to enhance communication and facilitate coordinated care.

• Engage with insurance companies for pre-certification and utilization management, ensuring that clients receive the services they need in a timely manner.

• Participate actively in discharge planning and aftercare planning meetings to ensure comprehensive support post-discharge.
\n
• Provide therapeutic meal support by exemplifying and promoting a healthy relationship with food for clients in our care.

• Deliver safe, empathetic, and affirming care tailored to the unique circumstances and intersecting identities of each client, fostering a sense of trust and security.

Patient Population / Client Focus\n

You will primarily work with patients facing serious health challenges and their families in a hospice setting. This requires not only clinical expertise but also immense compassion and respect for end-of-life experiences. Qualifications\n

• A Master's degree in Social Work from an accredited institution is required.

• A valid Licensed Clinical Social Worker (LCSW) license in the state of Arizona is necessary to practice.

Compensation and Benefits\n
