Job Details Seeking an experienced Registered Nurse to join a growing hospice and palliative care team as a Hospice Clinical Coordinator RN. Responsibilities include: Assess patients referred for hospice and palliative care services Coordinate hospice admissions and transitions of care Collaborate with physicians, case managers, social workers, and hospital staff Develop individualized care plans Provide symptom management recommendations Participate in interdisciplinary care planning meetings Educate patients, families, and referral sources regarding hospice services Maintain accurate clinical documentation Support inpatient hospice patients receiving General Inpatient (GIP) level care Schedule options: 4 x 10‑hour shifts (9:00 AM – 7:00 PM) 5 x 8‑hour shifts (9:00 AM – 5:00 PM) Requirements Active New York RN license 2+ years of RN experience Experience in Hospice, Palliative Care, Oncology, ICU, Critical Care, Med‑Surg, or related settings Strong communication and care coordination skills BSN preferred Jobot is an Equal Opportunity Employer.
