Unhoused Outreach Case Manager

Mother Teresa Shelter in Corpus Christi, Texas, is hiring an Unhoused Outreach Case Manager. This is a professional, part-time position requiring knowledge and experience in client-centered case management within the unhoused population. The ideal candidate must have the ability to deliver human services and work effectively with persons at various stages of the continuum of care of Mother Teresa Shelter (MTS), Mother Teresa Transitional House (MTTH), and Representative Payee program clients (Representative Payee).Ministerial Character:The Bishop is the visible principle and foundation of unity in the particular Church (The Diocese) entrusted to him. He shares with them the apostolic mission and entrusts various responsibilities to them.Each position employed in the Diocese of Corpus Christi helps to extend the ministry of the Bishop in particular ways as outlined in the job duties and responsibilities. Therefore, the employee in this position is closely connected to and assists the Bishop in the performance of his ministry and thereby engages in ministry for the church.Key Duties and Responsibilities:Under the supervision of the Operations Supervisor at Mother Teresa Shelter, Inc., the Unhoused Outreach Case Manager spends approximately 80% of their time providing outreach, awareness, and case management services to individuals who fall within the HUD definition of homelessness (literally defined, focused on unhoused persons outside of the MTS/MTTH network). The remaining 20% of time is spent managing the MTTH, overseeing operations, resident supervision, policy enforcement, maintaining building codes/regulations, and fostering a stable living environment. The list below reflects the general details considered necessary to describe the primary functions of the job identified and shall not be construed as a detailed description of all the work requirements that may be inherent in the job.As an integral part of the ministry for the Church, this position will support and uphold the philosophy of Catholic ministry and the mission of the Diocese of Corpus Christi in both professional and personal lifeActs as a witness to Gospel values by modeling the teachings of the Catholic ChurchContinuously grow in the ministry by participating in religious ceremonies, training sessions, and reflection activities as directedStreet Outreach for the UnhousedPerform literal street outreach and build rapport with individuals who fall within category 1 and category 4 of HUD defined homelessness (including but not limited to: help individuals access resources, transportation, documentation retrieval, provide referrals to community agencies, etc.)Conduct outreach and interviews to deem eligibility for potential residents of MTTHEnsure proper documentation of the case file in accordance with HMIS system or internal CRMS system of the agency; including maintaining general reporting as instructed by the Director of Mission AdvancementAddress client emergency and crises as needed.Maintain the confidentiality of all client data.All other duties assigned by the Executive DirectorCommunity Awareness/OutreachServe as the MTS representative providing awareness about the unhoused population, stay current with resources, and strengthen communications within the continuum of care network (including but not limited to: attending HIP events, participate in the city counting of the unhoused population, continuum of care street outreach efforts, etc.)Maintain a monthly calendar of all community partners/agencies who provide services at Mother Teresa Shelter in collaboration with the MTS Operations Supervisor and increase partnership opportunities in collaboration with the Director of Mission AdvancementProvide leadership and support to volunteer mentors working with un-housed clients.Participate in inter-agency case management collaborations to share information, resources and problem-solve client challenges and barriers.Facility ManagementResponsible for managing the day-to-day operations of MTTH, including resident/staff safety, cleanliness, code compliance, report incidents, and submit maintenance requestsIn collaboration with the Director of Operations, assess maintenance requests and recommend if the issue can be completed internally or through a third-party vendorMake regular inspections of the facility to ensure supplies are stocked, machines are operational, the facility is sanitary, etc.Manage service contracts with external vendors approved by the Director of OperationsMaintain a budget ledger of facility expenditures/revenueRespond to emergency calls to provided phone for emergencies after regular work hoursResident ManagementConduct final interviews to deem eligibility for potential residents of MTTHComplete clerical duties of maintaining lease agreements for all residents, collecting monthly rent, coordinating drug screenings, receiving/distributing mail, enforcing policies outlined in the resident lease agreement, etc.Coordinate with the Unhoused Case Manager to pick up the weekly mealsBuild rapport with MTTH residents and collaborate with the Unhoused Case Manager when applicable to provide information/updates on residents in compliance with confidentiality agreementsCollaborate with the Unhoused Outreach Case Manager providing information to include in the monthly calendar of community partners/agencies of servicesEnsure proper documentation in accordance with HMIS system or CRMS system; including reporting outcomes as instructed by the Director of Mission AdvancementMental/Physical Demands:Ability to carry, pull, push, lift, or move objects 25 pounds or moreAbility to maintain composure under duress; maintain emotional control under stressMust maintain confidentiality of the departmentMinimum Qualifications:Associate degree or related field experience with individuals experiencing homelessness, and/or mental health needs, substance abuse/dependency needs or co-occurring disordersKnowledge of local community resourcesStrong computer skills;
