Title: Case Manager II - Youth MICD Specialist

FLSA Status: Non-Exempt

Salary: Hourly

Purpose:

The Case Manager II is responsible for providing comprehensive, community-based services to youth (age 14-21) with severe and persistent mental illness (SPMI) and chemical dependency on Guild’s Assertive Community Treatment (ACT) team. This role collaborates within an interdisciplinary team to provide direct client support, manage medications, assist with benefits, and coordinate care, all aimed at improving client functioning and overall quality of life. The role is further specialized to focus on working with individuals with chemical dependency issues that are further complicating their mental illness and overall functioning. 

Key Responsibilities:

Duties and Responsibilities:

• Build trusting relationships with clients to encourage their active participation in treatment and recovery. 
• Assist clients in managing their mental health symptoms, daily living skills, and overall well-being. 
• Ensure timely medication delivery to clients and monitor adherence to prescribed treatment regimens. 
• Assist clients in renewing or applying for benefits, including Medicaid, Social Security, housing assistance, and other community services. 
• Collaborate with other service providers and natural supports, ensuring clear communication and coordination of services. 
• Actively participate in team meetings and care conferences to discuss client progress and share insights with team members.
• Complete Substance Use Assessments for the individuals currently on the ACT team and update yearly at minimum
• Educate and support clients and their families on signs, symptoms, and resources for people with dual diagnoses.
• Coordinate with outside inpatient services to detoxify clients and establish linkage to outpatient treatment, self-help programs, outpatient services and residential facilities.. 
• Serve as a consultant and educator to fellow ACT team regarding co-occurring disorders and chemical dependency.
• Complete required documentation in compliance with organizational and regulatory standards. 
• Perform other duties as assigned to support client care and department operations.

Required Skills and Abilities:

Knowledge:

• Understanding of mental health diagnoses, treatment modalities, and the specific needs of individuals with SPMI. 
• Understanding of community resources, county benefits, and mental health services. 
• Specific Training and knowledge in evidence-based practice for individuals with chemical dependency. 

Skills:

• Strong verbal and written communication. 
• Problem solving and independent decision making.
• Motivational Interviewing and other interventions to assist individuals through the stages of change. 

Abilities:

• Ability to manage a caseload, prioritize tasks, and meet deadlines in a dynamic, community-based environment. 
• Ability to assess and intervene in crisis situations, utilizing de-escalation techniques and connecting clients to higher levels of care when needed. 
• Ability to work effectively in a team-based environment and collaborate with multiple stakeholders to meet client needs. 
• Ability to maintain flexibility in prioritizing while working in a changing environment. 
• Ability to navigate stairways in buildings that do not have elevators. 
• Ability to lift and carry up to 50 pounds.

Working Conditions:

• This position is primarily community-based, requiring frequent travel to clients' homes and various community locations. 
• Occasional evening or weekend work.

Education and Experience:

Case Manager II

• Has a master’s or other graduate degree in behavioral sciences or related fields OR 
• Has a bachelor’s degree in one of the behavioral sciences or related fields and has 2,000 hours of experience in the delivery of services to persons with mental illness OR 
• Has 4,000 hours of experience in the delivery of services to persons with mental illness. 

Co-occurring Specialist

• At least one year of experience in substance abuse treatment workplace, a co-occurring program, or primarily with individuals with co-occurring chemical dependency. 
• Can provide or attest to substantial education or training in substances, chemical dependency, and the impacts on mental health disorders. 

Preferred:

• One-year work experience in case management or providing community-based mental health services.
• The co-occurring disorder specialist may also be an individual who is a licensed alcohol and drug counselor as described in section 148F.01, subdivision 5, or a counselor who otherwise meets the training, experience, and other requirements in section 245G.11, subdivision 5.

Additional Requirements:

• Must not be actively receiving services from Guild Incorporated and must not have received services within the last two years.
• A valid Class ‘D’ Driver’s License, own a reliable vehicle, a satisfactory driving record and necessary insurance.
• The ability to provide on-call crisis support 24/7 in weekly intervals on rotation.

The salary range for this position is between $27 - $35 hourly. The range reflects the minimum and maximum salary for the position. Compensation will depend on the candidate’s professional experience, skills, and relevant education, training, and certifications.
