Job Description
Title: Treatment Program Coordinator - Rehabilitation

Schedule: Days, 8 hours

Assignment Details:
- Guaranteed Hours: 40 per week
- Contract Length: 13 weeks

Roles & Responsibilities:
- Collaborate with an interdisciplinary team to plan and conduct segments of treatment programs aimed at restoring and enhancing physical, social, and mental functions.
- Serve as a co-leader in guiding patient groups to achieve departmental objectives.
- Assess patient needs and develop individualized treatment plans.
- Monitor patient progress and adjust treatment plans as necessary.
- Document and report on patient outcomes and program effectiveness.
- Educate patients and families about treatment options and strategies for improvement.
