Work Environment

This role is predominantly onsite in a home care facility, allowing you to make a significant impact in the comfort of patients' homes. You will work a full-time schedule of 40 hours per week, which may include variable weekends, as we strive to accommodate the needs of our clients while ensuring their recovery journey is as smooth as possible. Key Responsibilities

As a Physical Therapist at Renown Physical Therapy, your core responsibilities will include:

• Conducting thorough evaluations of patients to assess physical function, strengths, weaknesses, and limitations.
• Developing personalized treatment plans that address each patient's specific needs and goals, utilizing evidence-based practices.
• Implementing and teaching therapeutic exercises, manual therapy techniques, and modalities as part of the treatment process.
• Monitoring patient progress, making necessary adjustments to treatment plans, and re-evaluating patients on a regular basis.
• Documenting patient care activities, treatment plans, progress notes, and discharge summaries in compliance with legal and regulatory standards.
• Maintaining open communication with all members of the interdisciplinary team, including physicians and referral sources, to ensure comprehensive patient care.
• Participating in community outreach initiatives to promote health education and awareness.
• Mentoring and supervising support staff, students, or interns, enhancing the educational environment for those new to the field.
• Staying updated with industry standards through continuing education and participation in in-service training sessions.

Patient Population / Client Focus

In this role, you will primarily work with a diverse patient population, including individuals recovering from surgery, managing chronic pain conditions, or requiring rehabilitation post-injury. Your expertise will help guide patients through their recovery, utilizing techniques tailored to their unique circumstances, whether they are young athletes or elderly individuals seeking to regain mobility.

Qualifications

To be considered for this position, candidates must possess the following qualifications:

• A Bachelor's Degree in Physical Therapy from an accredited institution.
• Current state licensure in Physical Therapy to practice in New Jersey.
• BLS certification is required to ensure readiness in emergency situations.
• Strong written and verbal communication skills to connect with patients and collaborate with interdisciplinary teams.
• Proficient in using electronic medical record (EMR) systems and standard office software.

Preferred Experience

While a minimum of two years of experience as a licensed physical therapist is required, experience specifically within a home care environment is highly desirable. Familiarity with treatment protocols for varied patient populations will enable you to adjust your approaches effectively and improve patient outcomes.
