Work Environment

This position is primarily onsite, engaging directly with patients in their homes within the community of Woodbridge, NJ. Key Responsibilities

• Conduct comprehensive evaluations including detailed patient histories and physical assessments to formulate individualized treatment plans.
• Clearly communicate evaluation results and treatment plans to patients and their families, allowing them to participate actively in setting rehabilitation goals.
• Effectively implement evidence-based treatment protocols that address specific patient needs, adapting to their evolving conditions and the healthcare landscape.
• Collaborate with interdisciplinary healthcare teams to ensure a holistic approach to patient care, documenting any changes in patient condition requiring adjustments to treatment plans.
• Perform timely and accurate documentation of all patient interactions, maintaining compliance with healthcare standards and regulations, including progress summaries and discharge evaluations.
• Manage time effectively, adhering to productivity expectations and maintaining an organized schedule while participating in community outreach and marketing initiatives.
• Assist in developing departmental goals and objectives, adhering to state regulations and accreditation standards.
• Champion continuing education initiatives for self and staff, serving as a clinical instructor to help mentor students in the field.
• Promote outstanding patient experiences and improve access to care through proactive problem-solving and adaptability to meet patient needs.

Patient Population / Client Focus

The focus of this role is on patients recovering from orthopedic surgeries, sports injuries, and musculoskeletal disorders. You will work closely with individuals who require guidance in rehabilitating their physical capabilities and achieving optimal function. Our patients vary in age and physical conditions, requiring a tailored approach that addresses their unique needs and health goals.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution, with preference for a Doctorate degree in Physical Therapy.
• A valid state license to practice physical therapy within New Jersey.
• Current certification in Basic Life Support (BLS) from the American Heart Association.
• Dedication to ongoing professional development and competence in clinical practices.

Preferred Experience

• Prior experience in an orthopedic or home health setting is beneficial, though not required.
• Experience in using electronic health record (EHR) systems for documentation and patient management.
