Work Environment

This position will require you to work onsite in the patient's home environment. You will be closely interacting with a diverse patient population, fostering relationships built on trust and compassion. The daily schedule will be Monday to Friday, allowing for a healthy work-life balance while enabling you to engage directly with patients in their own spaces, adapting treatments to their unique environments and needs.

Key Responsibilities

• Direct the treatment of physical therapy services, aimed at preventing disabilities and relieving pain while focusing on restoring muscle function, in alignment with the personalized care plan developed for each patient.
• Supervise physical therapy assistants to ensure effective implementation of treatment plans, while providing guidance and support in clinical practices.
• Conduct thorough evaluations of each patient's level of function using diagnostic assessments and functional ability tests to inform individualized treatment plans.
• Assess the home environment for potential rehabilitation barriers and provide tailored recommendations that promote independence for the patient.
• Deliver prescribed physical therapy services in the home setting, which may include manual therapeutic exercises as well as instructing patients in functional activities.
• Administer modality treatments including traction, massage, and application of physical agents designed to improve patient outcomes.
• Evaluate and adjust prosthetic and orthotic devices as necessary, collaborating with physicians regarding modifications appropriate for each patient.
• Maintain accurate clinical records including initial evaluations, progress notes, and discharge summaries while adhering to regulatory and organizational standards.
• Work collaboratively within an interdisciplinary team, contributing insights during case conferences to ensure a holistic approach to patient care.
• Provide education and support to clients and their families, ensuring they are informed about their therapy programs and any assistive devices used.

Patient Population / Client Focus

The primary patient population for this role consists of individuals requiring orthopedic rehabilitation, including post-surgical patients, those recovering from injuries, and clients with chronic conditions affecting mobility. Your expertise will directly enhance their rehabilitation journey, focusing on restoring independence and improving overall health outcomes.

Qualifications

• A Doctorate of Physical Therapy (DPT) from an accredited institution.
• Current state licensure as a Doctor of Physical Therapy by the Physical Therapy Examining Committee in New Jersey.
• A minimum of two years of relevant clinical experience, ideally in a home care setting or with orthopedic patients.
• Current CPR Certification to ensure patient safety during treatment.

Preferred Experience

• Experience working with diverse populations and in various home environments requiring adaptable therapeutic strategies.
• Proficiency in electronic health records (EHR) systems to manage patient documentation efficiently.
• Familiarity with assistive devices and adaptive methodologies that promote rehabilitation for orthopedic patients.

Compensation and Benefits
