Licensed Orthopedic Physical Therapist - Home Care Specialist

Work Environment

This role is predominantly onsite, as the therapy services are provided in the homes of our clients across Old Bridge and surrounding areas. Key Responsibilities

• Perform thorough evaluations of patients requiring orthopedic rehabilitation, using standardized assessment tools to gauge their functional status and recovery potential.
• Develop and implement individualized treatment plans that incorporate a variety of therapeutic techniques, including exercise regimens, manual therapy, and utilization of specialized equipment to alleviate pain and enhance mobility.
• Utilize modalities such as ultrasound or electrical stimulation where appropriate, and educate patients on maintenance techniques and self-management strategies for long-term success.
• Monitor and document patient progress systematically, making necessary adjustments to the treatment plan based on client feedback and observed outcomes.
• Engage closely with patients and their families to deliver education about therapy processes, facilitate their understanding of rehabilitation goals, and provide training in the use of any assistive devices required for independent living.
• Participate in regular team meetings to discuss patient progress and collaborate with other health professionals, including occupational therapists, physicians, and social workers.
• Maintain professional development by attending workshops, conferences, and relevant continuing education courses to stay current on best practices and emerging therapies in physical rehabilitation.

Our therapists will work with a diverse range of clients, including athletes recovering from sports-related injuries, seniors experiencing mobility challenges, and individuals rehabilitating from orthopedic surgeries such as knee replacements or fractures. Qualifications

• Licensed Physical Therapist with a Doctorate in Physical Therapy (DPT) or equivalent degree.
• Current and valid State License in Physical Therapy for New Jersey.
• Demonstrated proficiency in written and verbal communication skills to effectively interact with patients, families, and healthcare colleagues.
• Basic proficiency in word processing and applicable clinical documentation software.
• Adaptable and flexible in work practices, with a readiness to approach challenges and modify treatment plans as needed.
• A genuine interest in patient care and a positive attitude toward teamwork.

Preferred Experience

• While prior experience in a similar role is appreciated, we encourage new graduates with a strong educational foundation and eagerness to learn to apply.
• Experience with home health care services is a plus but not a requirement.

Compensation and Benefits
