Work Environment

This role is centered around onsite home care, where you will visit clients in their residences. Key Responsibilities

• Direct and oversee individualized physical therapy treatment plans aimed at preventing disability, relieving pain, and restoring muscle function according to clients' capabilities.
• Conduct evaluations of clients' physical condition through diagnostic assessments, including muscle, nerve, joint, and functional abilities tests.
• Collaborate closely with physical therapy assistants and other health professionals to ensure effective implementation of care plans, supervising assistants as per state regulations.
• Assess the home environment for rehabilitation purposes, making recommendations to enhance independence and safety for clients.
• Administer therapeutic exercises, traction, massage, and physical agents to restore and maintain function, guiding clients through both manual and non-manual activities.
• Evaluate, fit, and adjust prosthetic and orthotic devices, working closely with physicians to suggest necessary modifications.
• Document clinical records comprehensively, including initial evaluations, care plans, progress notes, and discharge summaries in an accurate and timely manner using EMR software such as Web PT.
• Maintain ongoing communication with clients and families, providing education regarding therapy devices and programs.
• Engage in interdisciplinary case conferences to evaluate and revise treatment strategies collaboratively, ensuring holistic client care.
• Timely submission of required documentation, clinical data, and payroll orders according to the company's policies.
• Perform functional assessments onsite and regularly update the care plan based on the assessment of client needs and the utilization of community resources.
• Complete additional assignments as required to meet the organization's goals and support patient outcomes.

Patient Population / Client Focus

You will primarily work with clients requiring rehabilitation and support services following orthopedic surgeries, chronic pain conditions, and musculoskeletal disorders. Your ability to adapt treatments to individual patient needs will be crucial for promoting their independence and enhancing their functional capabilities.

Qualifications

• A Doctorate in Physical Therapy (DPT) is required.
• Possession of an active and impeccable Physical Therapist license in New Jersey, with no history of disciplinary actions.
• Exceptional proficiency in various manual therapy modalities.
• Demonstrated experience crafting and executing effective therapeutic exercise programs.
• Ability to utilize EMR software effectively for accurate documentation and patient management.
• Up-to-date knowledge of current physical therapy techniques supported by research.

Preferred Experience

• Prior experience in home health care or a similar setting is preferred.
• Knowledge of OASIS documentation standards is advantageous.
• Familiarity with interdisciplinary team dynamics and communication strategies.

Compensation and Benefits

The position offers a competitive salary ranging from USD 100,000 to 120,000 annually, commensurate with experience, accompanied by a comprehensive benefits package.
