Work Environment

As a Physical Therapist in our home care setting, you will work directly with patients in various locations throughout Keyport and surrounding areas. You will be responsible for creating a comfortable and engaging atmosphere for your patients, ensuring they feel valued and understood throughout their rehabilitation process. The position requires you to be adaptable and resourceful, providing therapy in diverse home environments while ensuring safety and effective treatment delivery.

Key Responsibilities

• Conduct comprehensive evaluations of patients' physical abilities, documenting their functional limitations and developing evidence-based treatment plans tailored to their individual needs.
• Implement therapeutic interventions, including manual therapy, therapeutic exercises, and modalities, adjusting treatment strategies based on patient progress and response.
• Monitor and document each patient's progress in compliance with regulatory standards, maintaining clear and accurate records of their treatment plans, progress notes, and discharge summaries.
• Develop and execute discharge plans that include recommendations for ongoing care, community resources, and strategies for maintaining physical health post-therapy.
• Engage with interdisciplinary teams, including physicians and other healthcare professionals, to provide coordinated care and address complex patient needs effectively.
• Participate in ongoing training and development to stay current with industry trends, treatment methodologies, and compliance regulations.
• Foster positive relationships with patients, families, and caregivers, enhancing their understanding of the treatment process and encouraging active involvement in rehabilitation efforts.

Patient Population / Client Focus

In your role, you will primarily focus on orthopedic patients who require rehabilitation following injuries, surgeries, or chronic conditions affecting mobility. This may include individuals recovering from joint replacements, sports injuries, or post-operative care. You will build rapport with patients of varying ages and backgrounds, ensuring that therapeutic interventions are sensitive to their personal circumstances and rehabilitation goals.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited institution.
• Valid Physical Therapist license for the state of New Jersey.
• Current certification in Basic Life Support (CPR) as recognized by the American Heart Association.

Preferred Experience

A minimum of two years of experience in physical therapy, particularly in an outpatient or home care setting, is preferred. Familiarity with orthopedic conditions and rehabilitation protocols will enhance your effectiveness in this role.
