Key Responsibilities

• Conduct thorough evaluations including detailed reviews of medical history and physical assessments to determine patient needs and establish individualized treatment plans.
• Explain evaluation findings, treatment options, and projected outcomes clearly to patients and their families, ensuring their involvement in goal setting.
• Exhibit proficiency in executing treatment protocols based on each patient's unique presentation, while staying informed about advancements in healthcare practices.
• Collaborate with other healthcare professionals when necessary and document any changes in the patient's condition that require adjustments to their care plan.
• Oversee discharge planning and provide necessary follow-up care, ensuring seamless continuity of care for patients.
• Accurately document patient care activities while adhering to regulatory requirements, including creating clear baseline and goal-setting notes.
• Maintain effective time management and meet productivity targets, while participating in departmental initiatives and community outreach programs.
• Engage in the professional development of yourself and others, including serving as a clinical instructor for students in training.
• Promote a positive patient experience through compassion and flexibility, reducing barriers to accessible care.

Patient Population / Client Focus

The ideal candidate will work directly with a diverse client population, focusing primarily on orthopedic cases. Patients may include individuals recovering from surgery, injuries, or chronic conditions. Your ability to offer personalized care in the home setting will be pivotal in enhancing their recovery and achieving functional goals.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program.
• A valid state license for physical therapy practice in New Jersey.
• Current certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association.

Preferred Experience

While previous work experience in a physical therapy setting is not mandatory, candidates with exposure to orthopedic rehabilitation or home care settings will have an advantage. A Doctorate in Physical Therapy is preferred but not required.
