Work Environment

As a member of our team, you will have the opportunity to work on-site in various home care settings within Metuchen and the surrounding areas. The typical schedule for this role is Monday through Friday from 8 AM to 5 PM, allowing for a balanced work-life dynamic while you make meaningful contributions to our patients' recovery journeys.

Key Responsibilities

• Conduct thorough evaluations of patient needs, reviewing medical histories and performing functional assessments to establish precise treatment plans tailored to individual circumstances.
• Implement and deliver customized therapeutic protocols based on evaluation results, ensuring that each treatment session aligns with best practices and the evolving healthcare landscape.
• Maintain accurate and up-to-date documentation of patient care activities, including initial evaluations, treatment progress, discharge plans, and follow-up recommendations, to create a comprehensive overview of the patient's journey.
• Collaborate closely with other healthcare professionals, coordinating care and referring patients to specialists when necessary to ensure optimal outcomes.
• Provide timely communication with patients and their families to build trust and promote engagement in the rehabilitation process, being attentive to their concerns and progress.
• Participate in ongoing training and development to stay abreast of the latest industry trends, techniques, and regulatory requirements that pertain to physical therapy.
• Engage in team meetings and continual improvement discussions to share insights, support one another, and enhance overall service delivery.

Patient Population / Client Focus

This role primarily serves individuals with orthopedic conditions needing rehabilitative services. You will manage a diverse caseload of patients ranging from postoperative orthopedic surgery recovery to chronic pain management, tailoring therapeutic strategies to each person's specific physical abilities and goals. By delivering compassionate care in the home setting, you will help patients regain independence and function effectively in their day-to-day lives.

Qualifications

• Bachelor's degree in Physical Therapy, with a Doctorate degree in Physical Therapy (DPT) preferred.
• Current and valid physical therapy licensure in the state of New Jersey.
• CPR certification is required, highlighting our commitment to safety and emergency preparedness.

Preferred Experience

• A minimum of two years of experience in orthopedic physical therapy, preferably with a focus on home care settings.
• Proficiency in utilizing modern therapy equipment and technology relevant to patient assessments and treatment interventions.

Compensation and Benefits
