Key Responsibilities\n

• Conduct thorough assessments to identify the mental health needs of patients and develop personalized treatment plans tailored to their individual situations.

• Facilitate effective communication among stakeholders, including supervisors, healthcare team members, clients, and their families to ensure consistent support and progress tracking.

• Provide a proactively organized schedule of appointments, home visits, and follow-ups to assess and monitor client progress, including any necessary documentation to maintain compliance with treatment plans.
\n
• Coordinate home visits when required, ensuring thorough documentation through chart reviews and data analysis to inform clinical decisions.
\n
• Implement program plans effectively and monitor their execution, collaborating with interdisciplinary teams to achieve optimal patient outcomes.
\n
• Participate in regular team meetings and contribute to discussions regarding patient care and treatment strategies.
\n
• Compile and document monthly and quarterly progress notes, highlighting changes in patient status and the effectiveness of interventions.
\n
• Organize training initiatives to ensure that both staff and volunteers are well-versed in program protocols and documentation processes.
\n
• Conduct follow-up interviews and additional assessments to ensure continued support for individual patient needs and adjust treatment plans as necessary.

• Act as a liaison between the practice and the wider mental health community, promoting collaboration and awareness of mental health issues within the community.

• Contribute to creating and refining training materials aimed at enhancing program quality and staff competency.

Patient Population / Client Focus\n

As a Licensed Clinical Professional Counselor at Wellbeing Counseling Center, you will work with a range of patient populations, including adults, youth, couples, and families facing various mental health challenges. Your expertise will allow you to design individualized treatment strategies that consider the unique backgrounds and circumstances of each client, promoting healing, growth, and resilience.

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a closely related field.

• Active and unrestricted licensure as a Clinical Professional Counselor (LCPC) in the state of Tennessee.

• A minimum of two years of professional counseling experience, preferably in a clinical setting.

• Demonstrated proficiency in utilizing Electronic Medical Record (EMR) software and other relevant technologies for effective patient management.

• Strong understanding of legal and ethical standards within the field of counseling.
\n
• Exceptional interpersonal and communication skills, enabling you to build rapport and trust with a diverse clientele.

• Proven ability to work collaboratively within a healthcare team environment.

Preferred Experience\n
