Key Responsibilities\n

As a Licensed Clinical Professional Counselor at Crystal Lake Families & Kids Services, you will:

• Provide expert guidance and therapeutic interventions to support the emotional, physical, and mental growth of patients.

• Conduct comprehensive patient intakes, focusing on diagnostic assessments and functional evaluations, and develop tailored treatment plans.

• Deliver brief but impactful mental health interventions for clients needing support, using evidence-based techniques.

• Work alongside primary care providers to assess mental health and psychosocial issues, recommending referrals to specialty services when necessary.

• Initiate follow-up communications with patients to monitor treatment progress and adjust strategies as required.
\n
• Develop and implement relapse prevention plans aimed at helping patients maintain stable functioning.
\n
• Document patient progress meticulously, ensuring compliance with agency guidelines and the standards of Federally Qualified Health Centers (FQHC).

• Advocate for patients' mental health needs while educating them on self-management and healthier lifestyle choices.

• Participate in interdisciplinary team meetings to evaluate and enhance behavioral health services provided.

• Engage with clinic management on behavioral health program evaluations and best practices.

Qualifications\n

Compensation and Benefits\n

Crystal Lake Families & Kids Services offers a competitive salary range of $87,000 to $105,000 annually, commensurate with experience and skills.
