Bilingual Discharge Planner: Care Coordination & Follow-Up

Prime Healthcare

Mercy Medical Center in Aurora, IL is seeking a Full-Time Social Work Coordinator to support discharge planning and post-discharge scheduling. The role coordinates follow-up appointments with our Primary Care Group and Multispecialty practices to ensure continuity of care after hospital discharge, working closely with Case Management and the clinical teams. A High School Diploma is required; an Associate degree or LVN license is preferred, along with hospital experience. #J-18808-Ljbffr Prime Healthcare
