Details Home Health setting providing personalized in-home care to patients throughout the community Day shift schedule, 8:00 AM-4:30 PM Requires EPIC experience Mileage reimbursement provided in addition to pay rate Requirements Doctor of Physical Therapy (DPT) degree required if licensed on or after January 2016, or foreign equivalent Physical Therapy license required in all applicable states of practice (license required if applicable) Current BLS certification required Valid driver's license required Must be at least 21 years of age or older Minimum 2 years of experience in specialty required, with recent experience within the last 3 months EPIC experience must be noted in employment history Additional Information Plans and conducts treatment programs designed to restore and improve physical, social, and mental function Serves as co-leader in an interdisciplinary approach to patient care Completes evaluations and establishes individualized treatment and discharge plans Provides therapy interventions using appropriate protocols and techniques Completes comprehensive and accurate clinical documentation within established deadlines Supervises assigned assistants Required vaccinations are a condition of employment, with medical and religious exemptions where applicable Accepts local candidates at the same rate as travel positions with no radius rule Candidates with more than 5 days of requested time off will be automatically declined Candidates with a prior adverse action on their license will not be considered American Traveler Job ID P-748628.
