Work Environment

This position is primarily onsite, as you will be visiting patients at home. Key Responsibilities

• Evaluate patients referred by physicians and develop individualized treatment plans that align with their specific needs and physician recommendations.
• Implement a variety of evidence-based therapeutic interventions at the patient's home, focusing on improving functional mobility and quality of life.
• Maintain open lines of communication with patients and their caregivers, providing education about treatment plans and encouraging their active participation in the rehabilitation process.
• Demonstrate commitment to excellent customer service, fostering positive relationships that contribute to overall patient satisfaction and engagement.
• Collaborate with an interdisciplinary team of health care providers to enhance patient outcomes and drive successful rehabilitation results.
• Meet or exceed minimum utilization goals and engage in ongoing professional development activities to stay current with advancements in physical therapy.

Patient Population / Client Focus

In this role, you will work with a diverse range of patients, including those with orthopedic conditions, neurological disorders, pediatric patients, and sports-related injuries. Your ability to customize treatment approaches for varied diagnoses will be key in effectively addressing their rehabilitation goals and aiding in their recovery.

Qualifications

• A Doctorate degree in Physical Therapy (DPT) from an accredited program.
• Current and valid Physical Therapy license in the state of Virginia.
• Bachelor's degree in Physical Therapy or a related field.
• Certification in Basic Life Support from the American Heart Association.
• Strong communication skills, both verbal and written, and proficient computer literacy.
• Capacity to build and maintain meaningful relationships with patients and their families.
• Demonstrated eagerness to engage with patients positively and contribute to their recovery journey.

Preferred Experience

While we welcome all qualified candidates, experience in a home care setting or background in specialized areas such as sports medicine or pediatrics will be beneficial. Knowledge of advanced therapeutic techniques and engagement in community outreach are also preferred.
