Key Responsibilities\n

• Conduct comprehensive assessments to evaluate patients' psychosocial needs and formulate suitable treatment or referral plans, which may include short-term counseling, crisis intervention, and ongoing case management.

• Guide patients in navigating the complexities of healthcare systems by assisting with the completion of forms for entitlements, supplemental insurance, and other resources aimed at reducing dependency on emergency assistance.

• Facilitate access to prescription cost assistance by connecting patients to indigent programs available through pharmaceutical companies and community resources, enhancing their capacity to manage treatment costs.

• Serve as an advocate for patients in dealings with community agencies, ensuring they receive the support necessary to thrive.
\n
• Maintain meticulous and up-to-date patient records, preparing necessary documentation, reports, and other written materials as required to ensure compliance and facilitate ongoing care.

• Organize transportation assistance for patients who need help in accessing mental health services, reinforcing our commitment to patient-centered care.

• Participate actively in clinical and team meetings, contributing insights and updates related to case conferences to promote a cohesive approach toward patient care.

Patient Population / Client Focus\n

The ideal candidate will work with a varied patient population, including individuals with diverse backgrounds and mental health challenges. Qualifications\n

• A Master’s degree in Social Work, Psychology, Marriage and Family Therapy, or Counseling is required.

• Possession of one of the following active independent licenses: Licensed Professional Clinical Counselor (LPCC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT).

• A minimum of two (2) years of professional experience in mental health counseling or a related field is mandatory.
