Key Responsibilities\n

• Conduct comprehensive assessments to identify the psychosocial needs of patients and develop personalized treatment plans that may include short-term counseling and crisis intervention strategies.

• Provide direct case management support, aiding patients in navigating entitlement processes, completing necessary forms, and securing supplemental insurance benefits that bolster their financial independence.

• Facilitating access to prescription cost assistance by connecting patients with resources provided by pharmaceutical companies and community organizations.
\n
• Act as a patient advocate, effectively liaising with community agencies to ensure patients receive the support they need for holistic care.

• Maintain meticulous and up-to-date patient records, including the preparation of reports and documentation required for insurance and clinical evaluations.

• Coordinate transportation assistance for patients when necessary, ensuring they have access to needed services and appointments.

• Participate in regular clinical meetings, collaborating with team members to discuss case management strategies and share insights from patient interactions.

Patient Population / Client Focus\n

As a therapist, you will work closely with diverse clients, providing them tailored support that respects their unique backgrounds and experiences. Qualifications\n

• A Master's degree in Social Work, Psychology, Marriage and Family Therapy, or Counseling is required.

• Possession of an active and independent license as either a Licensed Professional Clinical Counselor (LPCC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT).

• A minimum of two (2) years of relevant professional experience in a mental health counseling or therapy role, demonstrating proficiency in handling complex patient cases.

Preferred Experience\n

• Experience working with telehealth platforms and virtual counseling methodologies is highly desirable.

• Previous experience in case management or community agency liaison roles will be advantageous.

• Knowledge of Los Angeles community resources and services related to mental health care is a plus.

Compensation and Benefits\n
