Work Environment

This is an onsite position, and as a Licensed Physical Therapist specializing in home care, you will work directly in patients' homes, enabling you to provide personalized care tailored to their specific environments and needs. You will typically maintain a schedule of 40 hours per week, with the possibility of variable weekends, enhancing work-life balance. Key Responsibilities

• Conduct thorough examinations and evaluations of patients who demonstrate functional limitations and disabilities, using evidence-based techniques.
• Develop, implement, and adjust individualized treatment plans based on patient progress and compliant with state laws and professional standards.
• Regularly consult with patients, their families, and members of the multidisciplinary team to ensure an integrated approach to care and effective communication.
• Maintain accurate and timely documentation of all treatments and progress, ensuring transparency and compliance with regulatory standards.
• Guide and supervise support staff, including Physical Therapy Assistants (PTAs), aides, and students, providing mentorship and fostering a learning environment.
• Engage in community outreach, continuing education, and quality improvement initiatives to enhance departmental programs and patient care standards.
• Work collaboratively with healthcare providers to coordinate patient care that addresses transitions through various healthcare settings.

Patient Population / Client Focus

Your primary focus will be on a diverse patient population, ranging from children to elderly individuals with a variety of orthopedic and neurological conditions. You will be tasked with creating engaging therapy experiences that respect each patient's unique background, culture, and individual goals for recovery. By tailoring your approach to each patient's specific situation, you contribute significantly to their overall health and rehabilitation journey.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited institution is required.
• A valid State License in Physical Therapy is mandatory for practice in Florida.
• Basic Life Support (BLS) certification is required prior to employment.
• Strong written and verbal communication skills are critical to maintaining effective patient relationships and collaboration with the healthcare team.
• Proficiency in computer software and electronic medical record systems to manage patient documentation and treatment plans effectively.

Compensation and Benefits
