Key Responsibilities\n

• Provide one-on-one counseling sessions to support patients and their families in addressing adjustment issues, anxiety, trauma, and emotional well-being throughout the rehabilitation process.

• Collaborate closely with an interdisciplinary team to develop and implement individualized treatment plans that incorporate the psychosocial needs of patients.

• Guide patients in accessing essential community resources, including support groups, vocational rehabilitation services, and housing assistance, facilitating seamless transitions in care.

• Educate patients and their families about available community resources and aftercare options to ensure ongoing support following rehabilitation.

• Participate actively in discharge planning to help facilitate smoother transitions from the clinic to home or other care settings, ensuring adequate follow-up care.

You will work with a diverse patient population that includes individuals recovering from strokes, spinal cord injuries, orthopedic surgeries, and neurological disorders. Qualifications\n
