In this full-time position, the LCSW will play a critical role in providing tailored mental health support and interventions to individuals and families grappling with complex medical and psychosocial challenges. Work Environment\n

This position is primarily remote, which allows flexibility and work-life balance while you support clients from the comfort of your own home or co-working space. However, you will also have the opportunity to conduct home and community visits when necessary, allowing for a personalized approach to care. The selected candidate will also engage with team members virtually, ensuring collaboration and communication within our dedicated hospice center.

Key Responsibilities\n

• Manage a small caseload of clients, providing intensive case management services tailored to the unique needs of children and families.
\n
• Triage and address urgent care needs or tasks in a timely manner, ensuring clients receive necessary support without delay.
\n
• Assist families in navigating and completing required forms and applications to access essential services and benefits.
\n
• Work with families to identify and address Social Determinants of Health (SDOH) that impact their well-being.
\n
• Facilitate connections to community resources and services that enhance clients' quality of life.
\n
• Track referrals meticulously and provide follow-up to ensure successful service delivery.
\n
• Help patients schedule and attend medical and behavioral health appointments, enhancing adherence to care plans.
\n
• Provide comprehensive administrative support including records retrieval, eligibility screenings, goal tracking, and maintaining accurate documentation of all interactions in our electronic health record system.
\n
• Coordinate necessary care among interdisciplinary teams consisting of doctors, nurses, and community partners to ensure holistic care delivery.
\n
• Conduct home and community visits to clients when required, facilitating deeper engagement and support.
\n
• Document all client interactions and services as per organizational standards and maintain compliance with regulations.
\n
• Participate in internal and external meetings, contributing insights during case conferences and administrative discussions.
\n
• Engage in continued education and training, including health coaching and engagement techniques, to enhance your skills and contribute to team knowledge.
\n
• Submit timely and accurate reports on data as part of our commitment to quality improvement.
\n
• Provide constructive feedback on case management services and contribute to the development of quality improvement measures.

Patient Population / Client Focus\n

You will primarily work with children and families who face significant challenges related to mental health and social circumstances. This vulnerable population may experience a variety of psychosocial stressors, and your expertise will be crucial in providing the necessary support and interventions to help them navigate these difficulties effectively.

Qualifications\n

• Current and active LCSW license in Pennsylvania.
\n
• Bachelor's degree in Social Work; a Master’s degree in Social Work (MSW) from an accredited program is preferred.
\n
• A minimum of 2 years of clinical experience within mental health or social services.
\n
• Proficiency in evidence-based therapeutic interventions, including cognitive behavioral therapy (CBT), dialectical behavior therapy (DBT), and others.
\n
• Strong competency in case management, assessment, and crisis intervention techniques.
\n
• Excellent communication skills, both written and verbal, with the ability to foster relationships with clients and multidisciplinary teams.

Preferred Experience\n

• Experience working within hospice or palliative care settings is a plus.
\n
• Familiarity with electronic health record systems and data management tools.
\n
• Knowledge of community resources and services in the Beaver, Pennsylvania area.
\n

Compensation and Benefits\n

The compensation for this role ranges from $80,000 to $90,000 annually, commensurate with experience.
