Case Manager

We are seeking a Case Manager who is adaptable, solution-focused, and comfortable working in a fast-paced, high-needs environment.

What You'll Do:

• Manage a caseload of approximately 20–25 participants, providing short term, goal-oriented case management
• Complete intake assessments and develop individualized service plans focused on housing navigation, healthcare access, mental health support, income stability, and life skills
• Meet with participants regularly to track progress, address barriers, and support rapid stabilization
• Coordinate and refer participants to community resources (DPSS, SSI, healthcare providers, food assistance, housing programs, etc.)
• Respond to urgent and crisis driven client needs while maintaining firm, professional boundaries
• Support participants in developing routines, problem solving skills, and readiness for permanent housing
• Collaborate closely with on-site staff to ensure program compliance, safety, and continuity of care
• Participate in team meetings and trainings as required

What We're Looking For:

• Strong ability to function effectively in an on-site, high traffic environment with frequent interruptions
• At least 2 years of experience working with vulnerable populations, preferably in interim housing, homelessness services, shelters, or social services
• Clear communication, strong documentation skills, and sound judgment
• Emotional resilience and the ability to manage crisis oriented interactions
• Education in social work, human services, or a related field is preferred but not required

Pay Details: $23.00 to $26.00 per hour
