Key Responsibilities\n

• Deliver clinical behavioral health services in alignment with CHDC's advocacy for quality patient care, focusing on both short-term symptom management and long-term health behaviors.

• Assist mental health consumers in navigating the complexities of physical and behavioral healthcare systems, enhancing their access to integrated health services.

• Educate patients on available resources, services, and wellness practices to foster an environment of disease prevention and encourage healthier lifestyle choices.

• Contribute to multidisciplinary treatment team meetings, sharing insights that promote cohesive care strategies for individual patients.

• Ensure timely and accurate reporting of all crisis situations and unusual incidents to the Medical Director and immediate communication of psychiatric emergencies to the Chief Medical Officer (CMO).

• Maintain comprehensive clinical documentation in accordance with regulatory standards and internal policies, facilitating effective communication within the health team.
\n
• Participate actively in the Quality Improvement program, engaging in assigned training sessions and ongoing professional development.
\n
• Uphold all policies and procedures, demonstrating compliance with CHDC's commitment to serving patients through integrated Primary Care Behavioral Health models.
\n
• Utilize evidence-based screening tools to objectively assess the healthcare needs of clients and communicate findings effectively.
\n
• Collaborate with our Center of Excellence Team to offer specialized support for patients dealing with Opioid Use Disorder, implementing evidence-based practices in treatment planning.

Patient Population / Client Focus\n

This position primarily serves vulnerable populations who often face barriers to accessing quality healthcare, including individuals with behavioral health needs, chronic illnesses, and those at risk of substance use disorders. Your ability to provide empathetic, patient-centered care will be crucial in building trust and promoting patient engagement.

Qualifications\n

• A minimum of a master’s degree in social work, along with a valid Licensed Clinical Social Worker (LCSW) credential, is required to practice in this role.

• Demonstrable experience of at least 2-3 years in the social work field, particularly within settings collaborating with healthcare professionals.

• Proven ability to work collaboratively with other healthcare providers, social service agencies, and directly with clients to foster a holistic approach to care.

Compensation and Benefits\n

The salary for this role ranges from $90,000 to $100,000 annually, reflecting your experience and professional dedication.
