This role is primarily remote, allowing for a flexible work setting while maintaining close connections with our community health clinic operations. Key Responsibilities\n

As a Licensed Clinical Social Worker at Desta Care Therapeutic, your responsibilities will include:

• Acting as a liaison between clients, families, and outpatient providers to ensure comprehensive care coordination.

• Engaging with insurance companies for pre-certification processes and effective utilization management to facilitate access to services.
\n
• Participating in discharge and aftercare planning, ensuring that clients receive continuity of care after leaving treatment.

• Providing therapeutic meal support, when applicable, and modeling healthy relationships with food as a part of holistic treatment.

• Delivering safe, empathetic, and affirming therapeutic care tailored to the individual needs of clients, considering their unique identities.

Patient Population / Client Focus\n

Our clientele represents a broad spectrum, including individuals from various cultural, social, and economic backgrounds. Qualifications\n

To be successful in this role, candidates must possess:
