Key Responsibilities

• Conduct thorough evaluations of patients, reviewing pertinent medical histories and completing specific assessments to determine individual rehabilitation needs.
• Develop and communicate tailored treatment plans, involving both patients and their families in goal-setting discussions to ensure alignment with their aspirations.
• Implement effective treatment protocols, utilizing evidence-based practices that are adapted to each patient's condition and progress, reflecting changes in the healthcare landscape.
• Collaborate closely with a multidisciplinary team of healthcare professionals, recognizing any changes in patients' conditions that may require adjustments to care plans or additional consultations.
• Manage comprehensive discharge planning and follow-up care, ensuring timely and accurate documentation of patient care activities that provide a clear picture of each patient's journey.

Qualifications

• A Bachelor's Degree in Physical Therapy or a related field is required, with a Doctorate in Physical Therapy (DPT) preferred.
• Possession of a valid physical therapy license in the State of Florida.
• Current Basic Life Support (BLS) certification recognized by the American Heart Association or equivalent.
• Strong interpersonal and communication skills to effectively interact with patients, their families, and other healthcare professionals.

Preferred Experience

• A minimum of 2 years of relevant physical therapy experience in an outpatient setting is strongly preferred but not strictly required.
• Experience with electronic health record (EHR) systems is beneficial for effective documentation and patient management.

Compensation and Benefits
