Description

• Assess patients in both inpatient and outpatient environments and craft tailored treatment plans
• Implement therapeutic interventions aimed at improving mobility, strength, balance, and overall function
• Work collaboratively with a team of nurses, physicians, case managers, and other rehab professionals to drive optimal outcomes
• Guide patients and families through therapeutic milestones, discharge planning, and home exercise routines
• Maintain detailed documentation of evaluations, treatments, and patient progress according to facility and regulatory standards
• Engage in interdisciplinary conferences and care team rounds for coordinated care delivery

Education

• Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution

Certifications

• Current state licensure as a Physical Therapist
