Key Responsibilities\n

• Conduct thorough mental health assessments to evaluate patients' emotional, behavioral, and social functioning, ensuring accurate diagnoses and effective support strategies.

• Provide individual and group psychotherapy tailored to the needs of each patient, employing evidence-based therapeutic techniques to address conditions such as anxiety, depression, and PTSD.

• Collaborate with healthcare professionals to develop and implement individualized treatment plans that are culturally sensitive and consider the unique circumstances of each client.

• Maintain detailed and accurate patient records using EHR systems, ensuring compliance with all regulatory standards and organizational policies.

• Participate in regular case consultations and team meetings to discuss patient progress, adjustments to treatment plans, and interdisciplinary collaboration.

Patient Population / Client Focus\n

The primary patient population consists of individuals receiving hospice care, many of whom are dealing with significant mental health challenges related to terminal illness or grief. Qualifications\n

• A Master’s degree in Social Work from an accredited institution.
