This position is fully remote, allowing for flexibility in your work-life balance while still actively contributing to the essential services at our Hospice Center. Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to evaluate clients’ mental health concerns, personal strengths, and available resources, which guide treatment planning.

• Deliver individual, family, and group therapy employing evidence-based modalities such as cognitive-behavioral therapy (CBT), solution-focused therapy, and trauma-focused therapy.

• Create and manage individualized treatment plans, detailing measurable goals and tailored interventions that are regularly reviewed and adapted as needed.

• Work collaboratively with psychiatrists, psychologists, and other healthcare professionals to ensure holistic and coordinated care for all clients.

• Utilize crisis intervention techniques to address urgent needs, including risk assessments and safety planning for clients in distress.

• Maintain meticulous records and documentation in compliance with ethical standards and legal regulations to ensure proper case management.

• Facilitate psychoeducation classes, group therapy sessions, and support groups that nurture a sense of community and belonging among clients.

Patient Population / Client Focus\n

This role primarily involves working with clients who are navigating the complexities of mental health challenges in the context of hospice care.
