RN Case Manager

In this role, you will assess patient needs, develop and implement care plans, and collaborate with providers, payors, and community resources to support quality, affordable care. You will monitor clinical outcomes, close care gaps, and promote adherence to evidence-based guidelines. The position requires strong clinical judgment, excellent communication, and the ability to educate patients and families while advocating for services that improve health, reduce readmissions, and optimize resource use.
