Description

American Traveler is seeking a Home Health Physical Therapist with 1+ years of home health experience to serve patients throughout the Mokena/Frankfort territory.

Details

• Home health setting covering the Mokena/Frankfort area territory
• 13-week assignment on day shift, 8-hour shifts
• Provides age-appropriate care to patients ranging from pediatric to geriatric
• Mileage reimbursed at the standard IRS rate

Requirements

• Minimum 1 year of Home Health PT experience required
• Current BLS certification required
• IL state license required if applicable
• Valid driver's license and car insurance required
• COVID vaccination required, with no declinations approved
• OASIS and start of care experience strongly preferred

Additional Information

• Responsibilities include evaluating, planning, and implementing individualized physical therapy programs to improve patient mobility and independence
• Coordinates care with patients, families, physicians, and other healthcare staff in line with the hospital's core values
• Completes timely documentation of all patient care activities in the electronic medical record
• Must be able to safely transfer and position patients
• Local candidates within the service territory will be considered; those too far from the area may be redirected
• No more than 5 days of RTO may be requested during the assignment
