Key Responsibilities\n

• Plan, coordinate, and implement personalized support packages tailored to each client’s challenges and dependencies.

• Conduct in-depth interviews to assess the current conditions of service users, identifying their unique needs, strengths, and areas for improvement.

• Set measurable, individualized goals for treatment and monitor progress through regular evaluations and adjustments to care plans.

• Provide crisis intervention during sessions, equipping clients with strategies to cope with immediate challenges.

• Offer expert guidance and counseling regarding the best courses of action for clients during their treatment.

• Maintain thorough and accurate documentation of client records, reporting on their status and progress as necessary.

• Attend multidisciplinary case conferences, contributing insights to collaborative treatment plans and advocating for client needs.

Qualifications\n

Compensation and Benefits\n
