Work Environment

This role is primarily based onsite in the patients' homes, emphasizing the need for our therapists to be adaptable and capable of providing the best care in diverse settings. The work schedule is structured from Monday to Friday, with shifts typically from 7:30 AM to 4:00 PM or 8:00 AM to 4:00 PM. Working in home care presents unique challenges and rewards, allowing therapists to witness firsthand the progress of their treatment plans and the impact they have on patients' daily lives.

Key Responsibilities

• Conduct thorough evaluations of patients' physical capabilities and limitations upon initial visits to develop customized treatment plans.
• Utilize a variety of therapeutic techniques and modalities, including targeted exercises, manual therapy, and advanced rehabilitative equipment, to enhance patients' mobility and functional outcomes.
• Continuously monitor and document each patient's progress, regularly reassessing therapeutic goals and updating treatment plans accordingly to reflect changes in the patient's condition.
• Provide education and training for patients and their families on exercises, equipment usage, and mobility strategies that enhance independence and self-care.
• Collaborate closely with interdisciplinary healthcare teams, sharing insights and progress reports to ensure a holistic approach to patient care.
• Maintain professional competence through ongoing education, including attending workshops, participating in professional societies, and engaging with literature relevant to physical therapy best practices.

Patient Population / Client Focus

As a Home Health Physical Therapist, you will work with a diverse range of patients, including older adults recovering from surgery, individuals with chronic pain conditions, and those rehabilitating after strokes or debilitating injuries. Providing care in a home setting requires sensitivity to each patient's unique environment and needs, creating a trusting relationship that encourages compliance and promotes positive health outcomes.

Qualifications

• A Doctorate degree in Physical Therapy (DPT) is mandatory.
• Current and valid state licensure in Physical Therapy.
• A minimum of two years of direct patient care experience in a clinical or home health setting.
• Excellent verbal and written communication skills to facilitate patient education and team collaboration.
• Proficient in using electronic health record systems and basic computing skills for efficient documentation and reporting.
• Ability to work independently and manage unpredictability in patient needs and schedules.

Preferred Experience

• Experience with geriatric populations or chronic illness management is preferred.
• Familiarity with assistive devices and rehabilitative technologies.
• Previous experience in home health care settings is advantageous but not required.

Compensation and Benefits
