Title Center Medical Director Company Oak Street Health Location 1663 S Westnedge Ave, Kalamazoo, MI 49008 Hours Full-time, Monday - Friday, 8am-5pm Compensation Advanced Practice Provider (APP): $155,000 - $165,000 Physician (MD/DO): $265,000 - $275,000 Role Description The Center Medical Director (CMD) leads the clinical team at the Oak Street Health center, providing 80% clinical care and 20% administrative leadership. The CMD sets an example of thoughtful primary care, mentors and coaches providers, and participates in organization‑wide strategic initiatives. Core Responsibilities Support organizational strategy by collaborating with Practice and Outreach Managers to achieve objectives such as Best Care Anywhere and Exceptional Growth. Drive implementation of OSH strategic initiatives and cascade information to the care team while escalating concerns and ideas. Coordinate onboarding and ongoing support for new providers, model excellent medical care, and ensure adherence to evidence‑based guidelines. Maintain care model excellence, including daily huddles, patient care planning, admission prevention work, and documentation workflows. Teach and support implementation of the value‑based full risk model of care, assessing root causes when metrics are not met. Establish a culture of coaching, conduct performance reviews, develop goals, and address concerns with constructive problem‑solving. Celebrate successes, communicate improvement opportunities, and support a positive culture of joy in practice. Other duties as assigned. Qualifications Bachelor tax credentials: M.D., D.O., A.P.N., or P.A. graduates. Board certification required in Internal Medicine or Family Medicine for M.D./D.O.; Family, Adult, or Gerontology for A.P.N. Experience in clinical leadership roles, coaching providers, and using a metrics‑driven approach to care. Demonstrated commitment to older adults and low‑income patients. Comfort with ambiguity, strong problem‑solving skills, and the ability to remain calm under pressure. US work authorization. Experience 5+ years of outpatient practice preferred. Fellowship training in Geriatrics, additional professional degrees (M.B.A., J.D., M.P.H.), or prior executive roles welcomed.
