Key Responsibilities\n

• Conduct comprehensive assessments to evaluate clients' social, emotional, and environmental challenges, ensuring that you gain a holistic understanding of each individual’s situation.

• Develop tailored service plans with clear goals, interventions, and progress tracking, aligning resources and strategies to meet client needs effectively.

• Provide individual and family counseling aimed at navigating life transitions and overcoming mental health challenges, utilizing evidence-based practices to enhance therapeutic outcomes.

• Collaborate closely with clients to craft personalized service plans, connecting them with local community resources such as food banks, housing services, and mental health support groups.
\n
• Engage in crisis intervention by performing risk assessments and creating safety plans for clients in immediate distress, ensuring their well-being during critical situations.

• Partner with community organizations and agencies to coordinate services, serving as an advocate for clients to ensure they receive comprehensive care.

• Participate actively in community outreach activities, increasing awareness of Flourish Counsel Connect’s services and driving engagement with underserved populations.

• Attend case conferences, team meetings, and professional training sessions to promote continuous learning and teamwork among staff.

Patient Population / Client Focus\n

Your work will primarily focus on individuals and families facing multifaceted challenges, including mental health disorders, socioeconomic barriers, and crises stemming from homelessness or domestic violence. By providing compassionate support and thorough intervention strategies, you will help transform their lives, empowering them to navigate and overcome these adversities.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.
\n
• Current state licensure as a Licensed Clinical Social Worker (LCSW) is necessary for this role.

• Strong understanding of trauma-informed care practices and familiarity with diverse social and economic justice issues.

• Excellent communication skills, complemented by cultural competence and the ability to establish trusting relationships with clients from various backgrounds.

Preferred Experience\n

• Prior experience in a social service agency, community health, or similar environment is highly preferred.

Compensation and Benefits\n
