Service Coordinator

Company Description Alta California Regional Center (ACRC) is a private, non-profit organization that partners with the State of Californias Department of Developmental Services to support people with developmental disabilities under the Lanterman Act. ACRC serves individuals age three and older, helping them access needed services and supports in their communities. The organization also supports infants and toddlers from birth to 36 months who qualify for the California Early Start program and require early intervention services. ACRC focuses on person-centered planning, collaboration with families and providers, and promoting inclusion and quality of life. Role Description The Service Coordinator is a full-time, hybrid role based in Yolo, CA, with some flexibility for work from home. The role involves managing a caseload of individuals and families, conducting intake and assessment, and developing person-centered service plans in accordance with applicable regulations and ACRC policies. The Service Coordinator coordinates services among community providers, schools, healthcare professionals, and families, ensuring timely access to supports and monitoring progress toward goals. Daily responsibilities include maintaining accurate and timely documentation, participating in interdisciplinary meetings, responding to inquiries, and supporting individuals and families in navigating resources and advocacy. The role also involves regular communication by phone, email, and in person, as well as occasional community outreach and collaboration with other team members.

Qualifications

• Demonstrated Service Coordination skills, including case management, care planning, and resource navigation.
• Strong Interpersonal Skills and Communication abilities, with capacity to build trust, listen actively, and interact effectively with diverse individuals, families, and community partners.
• Experience in Operations Management or related tasks, such as organizing multiple priorities, maintaining documentation, and following regulatory and organizational procedures.
• Customer Service skills with a focus on empathy, problem-solving, and professional responsiveness to client and stakeholder needs.
• Bachelors degree in social work, psychology, human services, education, or a related field; relevant experience in developmental services or community-based programs is preferred.
• Knowledge of or willingness to learn about the Lanterman Act, California Early Start, and community resources for people with developmental disabilities.
• Ability to work both independently and collaboratively in a hybrid environment, with reliable transportation for in-person meetings and community visits as needed.
• Proficiency with basic office software and electronic records systems; strong organizational and time-management skills.
