Work Environment

As a Physical Therapist with Reliant Physical Therapy Clinic, you will work onsite, focusing primarily on home care. Key Responsibilities

• Conduct thorough evaluations of patients' physical capabilities by reviewing their medical history, performing specific assessments, and documenting results to inform treatment planning.
• Clearly communicate findings from evaluations and treatment recommendations to both patients and their families, involving them actively in the goal-setting process.
• Implement individualized treatment protocols, continuously adapting them based on patient progress and changes in condition as guided by evidence-based practices.
• Collaborate with other healthcare providers to ensure comprehensive care, making referrals when necessary for specialized medical or rehabilitation consultations.
• Develop discharge plans that are patient-centered, providing clear follow-up care instructions and necessary referrals to support ongoing recovery.
• Maintain accurate and timely documentation of all patient interactions, proactively tracking progress and treatments in compliance with healthcare regulations.

Patient Population / Client Focus

The patients you will serve through our home care program include a diverse range of individuals recovering from surgery, injury, or chronic illnesses. Qualifications

• A Bachelor's or Doctorate degree in Physical Therapy is required.
