Pay Rate: $52/hour
Sign-On Bonus: $6,000 (Eligible newly hired nurses receive $3,000 after 6 months and $3,000 after 1 year of continuous employment.)

Job Overview

We are seeking a compassionate and dedicated Registered Nurse (RN) to join our team. In this role, you will provide skilled nursing care to patients in their homes, coordinate multidisciplinary services, develop individualized care plans, and support patients in achieving optimal health outcomes.

Key Responsibilities

• Perform comprehensive patient assessments and develop individualized care plans.
• Complete OASIS assessments and maintain accurate EMR documentation.
• Coordinate patient care with physicians, therapists, and interdisciplinary healthcare teams.
• Monitor patient progress and communicate changes in condition to the care team.
• Educate patients and families on disease management, medications, and self-care.
• Supervise Home Health Aides (HHAs) and complete required supervisory documentation.
• Manage an assigned home health caseload throughout Manhattan.
• Ensure compliance with Medicare guidelines, agency policies, and regulatory standards.

Qualifications

• Active New York State Registered Nurse (RN) License .
• Graduate of an accredited School of Nursing (BSN preferred).
• Minimum 1 year of hospital RN experience or 1 year of Certified Home Health Agency (CHHA) experience preferred.
• OASIS documentation experience preferred.
• Strong assessment, communication, and care coordination skills.
