Licensed Practical Nurse (LPN) - Part-Time and Full-Time Opportunities

We are seeking dedicated and compassionate Licensed Practical Nurses (LPNs) to join our dynamic care team at Southbridge Rehabilitation & Health Care Center! You will play a vital role in providing high-quality care and customer service to our patients and their families.

Openings include:

• 16 hours on 7a-3p
• 24 hours on 11p-7a

Weekly pay ranges from $35 to $40 an hour, depending on the shift. Evenings, overnights, and weekends have shift differentials.

Responsibilities:

• Provide direct nursing care under the supervision of the RN to residents and supervise the day-to-day nursing activities performed by the nursing assistant
• Maintain current resident care plans and follow physician's orders
• Transcribe and review patient records daily
• Document comprehensive and complete nursing notes
• Visit each resident daily to evaluate resident needs and maintain accurate awareness of residents on alert list
• Administer medication according to procedure
• Demonstrate correct safety techniques and respect facility equipment and supplies
• Contribute to effective communication and pleasant working conditions
• Comply with facility Privacy Practices Policies & Procedures, protecting residents' individual health information

Requirements:

• Graduate of an approved LPN/LVN program
• CPR certified
• Current, unencumbered license to practice as an LPN/LVN in Massachusetts
