Bilingual Analyst, Case Management

Responsibilities Assessing, planning, implementing, and coordinating all case management activities with members. Managing a caseload that includes supportive and medically complex members. Developing a proactive course of action to address issues presented to enhance member's overall wellness. Determining appropriate services and supports due to member's health needs. Requirements Bilingual: English, and Spanish. Must reside in zip code 33184, 33172, and 33186. Willing and able to travel 50-75% of their time to meet members face to face in Miami Dade county and surrounding areas in FL. 1+ year of experience in behavioral health or a long-term care setting.
