HIV Medical Case Manager — Integrated Care & Resources

Jobtailor in Chicago is seeking a skilled Medical Case Manager to support HIV+ clients with complex needs. You will coordinate medical services, advocate, refer to resources, and enroll clients in benefits to improve health outcomes. You will link health care with psychosocial supports, monitor treatment progress, and ensure timely access to appropriate services.
