Job Description:

Job Summary:

As an Evening Shift LPN, you will play a vital role in providing high-quality care to our residents. You will work collaboratively with our interdisciplinary team to develop and implement individualized care plans, administer medications, and ensure the overall well-being of our residents.

Responsibilities:

• Provide direct patient care to residents, including:

  • Administering medications and treatments as prescribed

  • Assisting with activities of daily living (ADLs) such as bathing, dressing, and grooming

  • Monitoring vital signs and reporting changes to the nursing team

• Document resident care accurately and thoroughly in accordance with facility policies and regulations

• Develop and implement individualized care plans that address the unique needs and goals of each resident

• Participate in interdisciplinary team meetings to discuss resident care and develop strategies for improvement

• Maintain a clean, safe, and organized work environment that promotes a positive and respectful atmosphere for residents and staff

Requirements:

• Current LPN license in the state of Florida

• CPR certification

• Ability to work effectively in a fast-paced environment

• Strong communication and interpersonal skills

• Ability to work independently and as part of a team

• High school diploma or equivalent required
