Evening Shift LPN
Job Summary:

As an Evening Shift LPN, you will play a vital role in ensuring the health and well-being of our residents, providing high-quality care and support to meet their unique needs.
Responsibilities:
Provide direct patient care to our residents, including administering medications, wound care, and vital sign monitoring
Develop and implement individualized care plans in collaboration with the healthcare team
Promote a safe and clean environment by adhering to infection control protocols and maintaining a clean and organized workspace
Communicate effectively with residents, families, and healthcare team members to ensure seamless care coordination
Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
Assume responsibility for specific patient care areas, including vital sign monitoring, medication management, and documentation
